We are looking for empathetic, driven individuals with excellent communication skills and the ability to manage their own schedules while delivering deeply meaningful support to aging adults and their loved ones. - 5+ years of experience in a healthcare, elder care, or social services setting
- Clinical credentials such as LSW, LCSW, RN, LPN, or NP are a strong plus
- Bachelor’s degree required; Master’s degree preferred
- Strong communication skills and compassionate bedside manner
- Self-starter who thrives in flexible, independent work environments
- Proficient with Microsoft Office and web-based CRM systems
- Prior experience touring assisted living, memory care, or rehabilitation facilities is helpful but not required
- Guide families through the process of transitioning a loved one when home is no longer an option
- Consult with clients to assess needs, constraints, and care goals
- Identify and present a short list of options tailored to the individual situation
- Attend in-person tours and meetings to offer clinical and emotional support
- Serve as an ongoing advocate post-move-in through our Clinical Advocacy and Concierge Medicine services
- Build and maintain referral relationships with healthcare professionals
- Represent Veramazo at educational events and workshops
- Maintain up-to-date documentation and client notes in our CRM
